Q: Where can I get a copy of my medical records?
A: To obtain a copy of your medical records, please call the Medical Information Department at (317) 736-3573. You will be required to fill out a release of information when you pick up your records.
 
Q: How can I get a copy of my birth certificate/death record?
A: Call the Johnson County Health Department at (317) 736-3775. Birth Certificates and Death Records are not available through the hospital.
 
Q: I have a billing question. What should I do?
A: Contact the Patient Accounts Department at (317) 738-7878 or email questions to billrequest@johnsonmemorial.org.
 
Q: Does JMH have a gift shop?
A: The JMH Gift Shop is located on the first floor, by the outpatient services entrance. The JMH Gift Shop has a wide variety of items including flowers, cards, magazines, figurines and personal care items. Hours of Operation:
Monday – Friday 8 a.m. – 4 p.m.
Saturday – Sunday, Noon – 4 p.m.
