How do I redeem a patient and customer reward certificate?

Your patient or customer will present their My JMH Rewards certificate at the time of
purchase. All you have to do is redeem the certificate by completing one of the
options below:

• Redeem by calling 888-956-6430.

Once we have received your redemption information, a check will be mailed to you for
reimbursement of the $20.00 face value of My JMH Rewards certificate.
