At Johnson Memorial we want to ease your health care costs. That's why we have introduced a program designed to help you earn valuable rewards to use as cash at participating provider locations.

How does the program work?

• Simply pay your hospital bill in full, by the date indicated and earn five (5) points for each dollar spent.
• Accumulate a total of 2000 points, i.e. pay a total of $400 in patient out of pocket expenses, and we'll mail you a certificate worth twenty (20) Health Rewards dollars that you can use at any participating provider location.
