Our laboratory, located on the Johnson Memorial Hospital campus, offers a selection of affordable laboratory tests designed to evaluate your current health status. Payment is due at the time of service. Master Card, Visa, Discover are accepted. Medicare, Medicaid and private insurance cannot be billed. A physician order is not required.

Direct Access Lab Form and Charges

Lab Tests Procedure

All tests results will be mailed directly to your home address listed on the Direct Access Test Request form. However, you may come back to the hospital and get your results. You must present with a picture ID and sign a Request and Authorization for Disclosure of Medical Record Information form to pick up your results. No results will be released via telephone or faxed.

Please note: Direct Access testing should not replace care and treatment from your physician. At no time will a Laboratory employee provide any type of diagnosis related information to a patient. You are responsible for sharing direct access test results with your physician to obtain appropriate diagnosis.

In the event one of the tests selected through Direct Access Testing indicates a critical value (a reading that could immediately impact your health status), you will be contacted via a phone call. You will then be responsible for contacting your physician or medical provider with the results for follow-up care.

Before You Arrive

You will need to print the form and present it to the cashier for payment prior to receiving service in the laboratory. You must obtain a receipt of payment and present the receipt to the Laboratory at the time of service.
