The Patient Advocate program at Johnson Memorial Hospital is designed to give patients a tool for addressing any concerns or questions they may have about the care they received.
To contact the Patient Advocate, describe your concern or questions in the contact form shown below. When completed, select the “Submit” button. The message will be sent directly to the Patient Advocate office and you will receive a response the next business day.
