How to Access My JMH Patient Portal

Patients using the portal can view:

  • Health record and history
  • Lab results including microbiology
  • Medical imaging reports
  • Upcoming appointments/request appointments
  • Message your physician and office staff

*Note:  Please allow 36 hours before test results are available to be viewed on your portal. 

Birth Certificates and Death Records are not available through the hospital. Call the Johnson County Health Department at 317.736.3775.

The My JMH Patient Portal gives health providers access to a shared patient chart from all locations, whether on the hospital campus or at primary care centers throughout the county.The comprehensive chart will include information about the patient’s medical history and it can accompany you to different care venues, reducing the opportunity for miscommunication or errors.

How to create your new profile in the MyJMH patient portal

Enroll and create your profile

Step 2

Enroll by answering the following questions:

• First name:

• Last name:

• Date of birth:

• Social Security #: xxxxxxxxx (no spaces or dashes)

Step 3

Select “create your account” and follow the instructions.

Step 4

Click on to access your portal once you have created your account

Once you have successfully created your account, you will need to log on to access your portal. If you have trouble creating or accessing the portal please call 317.346.2861.

Portal Transition

Access to Previous Portal: Johnson Memorial has closed portals created prior to August 1, 2018.

Enroll in New Portal: Self-enrollment is available for patients who are age 18 or older who don't currently have access to the Johnson Memorial Hospital Portal. If you manage the health of a patient, talk to the patient's health care provider during the next visit to receive a personal invitation to the Johnson Memorial Health Portal. Self-enrollment is not currently available to caregivers or guardians.

Previous Medical Records: If you utilized our previous patient portal and need your full medical records, please contact our Release of Information Office at 317.736.3573.

How to Get a Copy of Medical Records


To obtain a printed copy of your medical records, please download and complete the HIPAA compliant Release of Information Authorization Form. Submit the form and a copy of a valid photo ID to the Medical Information Department at Johnson Memorial Health by fax at 317.736.3368 or by stopping by our office on the second floor of 1125 West Jefferson Street, Franklin, IN between 8 a.m. and 4 p.m. Monday through Friday, excluding holidays.
